Album Design Completion Form

This form must be filled out and submitted before we process each album you submit.
(One form per album)

Important Notice:
This form is in addition to the cover setup form found with the album designer.
When you submit your album from the album designer you should get a confirmation page saying your album was successfully submitted. If you did not get the confirmation page, your album will not be processed. If the confirmation page does not appear, resubmit your album again. If that fails, please contact us so we can be sure your album is processed correctly.

I have read and checked each item listed below in acknowledgement of understanding and accomplishing each step in completing my album design.

  1. I have read, understand and agree to the Album-FAQs
  2. I have scheduled and completed the Album Design Tutorial with Coolwater Studios and I can and will contact them with any questions I might have, free of charge.
  3. I have filled in the online album cover designer form and I have checked it for accuracy BEFORE I submitted my album for processing.
  4. I am aware of the "Image Validation Options" in Album-FAQs and I accept the images I've chosen for my album as they are. I agree that, upon receiving my album, if I don't like a print in my alum, I will pay for replacing any images I am not happy with including all costs for replacing images, taxes and shipping charges.
  5. I understand that albums are designed using the information I provide in the online album cover design form and that I will pay for fixing any errors or omissions including additional taxes and shipping charges.
  6. I understand that the images I've selected are final and accept their quality and I know if I have any concerns about any image I can ask CWS to send me a proof to validate my selection(s) BEFORE selecting "process album". If I'm not satisfied with the quality of the print after receiving my album I will pay for replacing it and all S&H charges. I understand that prints, albums or prints in albums are not refundable.
     
  7. I have printed out my album design using the report tool in the album designer.
          (Just in case I need to review the design)
  8. I understand that it can take from six to seventeen weeks to get my album once it has been processed and if there are any fixes or changes it can double this time.
  9. I understand that Coolwater Studios priority is preparing for and shooting events and this can delay album processing for an additional week or two. (We understand our customers want their albums ASAP and we do everything humanly possible to make this happen).
  10. I understand that after I process my album images the balance of my account must be paid before my album prints are sent to the album maker. 
  11. I understand that items marked with prices on this form are additional costs above the album that comes with my package.

I am a Boston, Massachusetts photographer and wedding photojournalist who specializes in wedding photojournalism, documentary wedding photography, with a fashion style that captures in photos the true nature and character of any person or event. I enjoy shooting reportage wedding photography, senior portraits, Bar Mitzvah and Bat Mitzvah photography, children's and family portraiture and documentary with a fashion tone wedding photography not only in Boston but also in Rhode Island. It is my goal to provide you with high quality wedding photography created with the greatest level of professionalism that a wedding photographer can achieve in Boston, Massachusetts and Rhode Island areas. I have had the pleasure of photographing many different ethnic ceremonies. I have been a Protestant wedding Photographer, a Catholic wedding Photographer, a Jewish wedding photographer, Greek wedding photographer, Indian wedding photographer, Cambodian wedding photographer, Chinese wedding photographer and same sex wedding photographer.
